Begin your journey toward high performance with the Efforts to Outcomes (ETO®) Performance Model designed specifically for programs implementing the evidence-based Healthy Families America (HFA) home visiting model for families with young children. HFA, created by Prevent Child Abuse America, aims to “promote child well-being and prevent the abuse and neglect of our nation’s children through home visiting services.” This PSM was developed by the California Home Visiting Program (CHVP), part of the Maternal, Child and Adolescent Health Division of the California Department of Public Health. It was developed as part of the CHVP’s successful efforts to create a standardized performance management system for all its HFA sites, funded under the federal Maternal, Infant, and Early Childhood Home Visiting (MIECHV) Program.

The California Department of Public Health is generously allowing Social Solutions’ customers to make use of the configuration, including all the work put into a Data Collection Manual, which provides all the paper forms used in California, and exact instructions for how data should be collected to assist with uniformity. The PSM also includes a number of reports, which have been reviewed by the HFA National Office for agreement with accreditation standards.

The participant dashboard provides an overview of contact information, family members and recent work, and offers a quick way to record new efforts.
