“Outcomes” is the buzz word of the day, but determining how to measure the benefits of program services, especially in the field of highly individualized senior case management, is not always easy.

This session explores one agency’s shifting emphasis from counting services alone to the more meaningful measurement of those services’ impact on each client.

The agency shares what they learned about their own performance, and offers tips about how to go about defining an outcome framework, implementing performance tracking software and managing the organizational change that is an integral part of the process. 

Participants will learn how one agency shifted their data tracking emphasis from counting services to measuring meaningful outcomes in practice.

Participants will understand three key requirements for successful outcome tracking in any agency: 

  1. Defining an outcome framework
  2. Implementing performance tracking software
  3. Managing organizational change
